Info gathering form for Service Projects at GWS2011 Conference

This form is to help us gather further information about the Service Project (SP) you will be hosting for the GWS2011 Conference. We will use this information to describe your SP on the website. In essence, this information is your "pitch" to GWS2011 attendees to participate in your SP. Therefore please phrase your answers with GWS2011 attendees in mind.

If you are involved in proposing more than one SP, please fill out a form for each one.

If you have co-organizers for your SP, please list them and their contact info here.

Please give a short (no more than 15 words) title for this SP that clearly states the main task to be accomplished.

Please indicate if your SP is a full-day or half-day project.

Note: half-day SPs will be scheduled for Wednesday afternoon, March 16, 1:30-5:30pm. Full-day SPs will be scheduled for Friday, March 18, 8:00am-5:00pm.

Please indicate whether your SP is "Consultative" or "Hands-on."

A Consultative SP is one in which you are asking GWS2011 attendees to offer their expertise to help you plan your project. Please indicate whether your Consultative SP is "One-Time Only" — meaning you are asking GWS2011 attendees to meet with you just once for a specified number of hours while they are in New Orleans, with no follow-up asked for or expected — or whether you are asking for an "Extended Commitment" in which GWS2011 attendees both meet with you for a specified number of hours while they are in New Orleans AND will engage in follow-up meetings via conference call or email.

A Hands-On SP is one in which you are asking GWS2011 attendees to "get their hands dirty" and help you work on a specific task that involves physical labor for a specified number of hours while they are in New Orleans.

If you checked "Consultative (Extended Commitment)" above, please describe the kind(s) of additional activities you are asking GWS2011 partiicipants to commit to, and the length of the commitment. For example, the description could read something like: "After the initial consultation in New Orleans, we will need participants to continue to work with us for approximately 6 months, during which time we will hold conference calls, interact through email, and ask participants to help draft and review a project master plan."

If you marked "Hands-On" above, please indicate whether you will supply all needed tools and equipment (including such items as work gloves & safety glasses) or whether participants will need to bring some or all of their own. Also indicate what type of clothing participants should wear.

Give the minimum number of GWS2011 participants you would need in order to effectively carry out the SP.

Give the maximum number of GWS2011 participants you can accept in order to effectively carry out the SP.

In 400 words or less, please describe your SP. This is the "pitch" that will appear on the GWS2011 conference website, and attendees will use it to decide whether they want to volunteer for your SP. Be as specific as possible. Include the following:

* Statement of need: why is this work needed?
* Goal: what do you want to achieve, and how will the communities you deal with benefit?
* Activities: what exactly will you be asking attendees to do? If yours is a "Hands-On" SP, indicate the general level of difficulty of the physical labor involved.
